Community Support
1. Care Coordination and Case Management - Our Core Available to all clients regardless of housing status. Led by CHWs and Resource Facilitators under licensed clinical oversight.
2. DAP Core Stability Support Community based coordination and resource navigation for individuals who need lighter touch support. A standalone service line separate from Senior Stability.
3. Senior Stability Program - Our comprehensive housing stabilization program for medically vulnerable seniors experiencing homelessness or housing instability. The Senior Stability Program includes three interconnected components.

Care Coordination and Case Management Intensive individualized case management, healthcare navigation, benefits enrollment, and ongoing support for every Senior Stability client.

Harbor House Short term stabilization housing for seniors who cannot safely access traditional shelters. Harbor House is the crisis entry point of the Senior Stability Program.

Permanent Supportive Shared Housing for the Aged Long term affordable shared housing for seniors on fixed incomes awaiting subsidized placement. The step down housing option within the Senior Stability continuum.

Care coordination is the foundation of everything we do at DAP. Our team of Community Health Workers, Resource Facilitators, and licensed clinical staff work alongside clients to navigate complex healthcare systems, coordinate medical appointments, enroll in benefits, manage chronic conditions, and connect to community resources. This is where DAP started and it remains the heart of our model.

COMMUNITY WALK-IN RESOURCE CENTER
Every Tuesday 1:00pm to 4:00pm No enrollment required. No appointment needed. Open to anyone in the community.
Services available during walk-in hours include benefit navigation, housing application assistance, resource connection, and general support navigating healthcare and social service systems. Anyone who needs help is welcome to walk through the door.No Appointment. No Enrollment. Just Help.
Every Tuesday from 1:00pm to 4:00pm DAP opens its doors to anyone in the community who needs assistance. No appointment is necessary. You do not need to be enrolled in any of our programs. You do not need a referral.
You simply need to walk in.

Licensed nursing services for medication management and medication set up. Billable through insurance. Available to clients who need clinical support managing complex medication regimens.

In home crisis support services for individuals experiencing acute destabilization at home. Details to
