DAP Services and Resources is a care coordination and housing stabilization organization serving medically vulnerable seniors and individuals with chronic health conditions in the Lansing area. We meet people where they are whether they need help navigating healthcare systems, accessing benefits, or finding a safe place to live.
DAP Services and Resources was founded in October 2021 by Yolanda Pope, a Licensed Practical Nurse with more than 15 years of experience in healthcare, senior living, and care coordination.
What began as a care coordination and case management organization serving seniors and medically complex individuals in the Lansing area quickly revealed a deeper truth about our community. Seniors were being stabilized through services — connected to healthcare, enrolled in benefits, supported through crises — only to lose everything because they had nowhere safe to live. The housing piece kept being the missing link.
So DAP built it.
In 2025 DAP launched the Senior Stability Program, opened Harbor House stabilization housing, and developed Permanent Supportive Shared Housing for the Aged. In 2026 DAP added community walk-in hours, expanded nursing services, and began development conversations for a purpose-built Senior Housing Stabilization Campus in Lansing.
Today DAP operates multiple programs, employs a team of Community Health Workers and Resource Facilitators under licensed clinical oversight, and has been recognized by the National Association of Housing and Redevelopment Officials for innovation in senior housing stabilization through our partnership with the Lansing Housing Commission.

Care Coordination Is Our Foundation
Before DAP ever opened a housing program we were walking alongside seniors and individuals with chronic health conditions helping them navigate some of the most complex systems imaginable. Healthcare. Insurance. Benefits. Medications. Family dynamics. Discharge planning.That work is still at the center of everything we do.
Whether a client needs help understanding a diagnosis, enrolling in Medicaid, coordinating between multiple providers, or simply having someone in their corner who speaks the language of the healthcare system DAP's care coordination team shows up.
Our housing programs exist because our clients need them. But our care coordination services exist for everyone housed or unhoused, stable or in crisis.
